How to become a Packard Health patient
Packard Health is a family practice, not a walk-in or urgent-care clinic. We want to establish an ongoing relationship with you to help maintain your health over time. Please use the following guide to begin your relationship with Packard Health.
If you are covered by a health plan:
- Find out if you have an HMO or another type of plan that requires you to choose a primary care physician (a primary doctor).
- If your plan does not require you to designate a primary care physician (PCP), simply call Packard Health to schedule a New Patient appointment. Caregivers may call on behalf of patients who are unable to call themselves.
- If your plan does require you to designate a PCP, choose from the list of providers below and call your health plan to "transfer your care" to Packard Health. Ask your health plan for the date that this transfer will become effective, and then call Packard Health to schedule a New Patient appointment on or after that date.

If you are not covered by a health plan:
- Schedule a meeting with our patient advocate, who will help you determine whether you are eligible for any health coverage. You will need to bring in recent pay stubs, unemployment checks or your tax return, and you will need to fill out one or more forms.
- If you are not eligible for a health assistance program and you pay for your visit in full on the same day, you will receive a 20% discount on the cost of that visit.
