What to expect on your first visit to Packard Health
On the day of your appointment:
- Plan to arrive promptly to your first scheduled appointment. If you are late, you may be asked to reschedule or to wait until your provider is available.
- Don't forget to bring:
- Any forms you received from us in the mail, completed
- Your insurance card
- All medications you (or the patient) are currently taking
- The name and address of your previous doctor
- Immunization records, if the appointment is for your child
When you arrive:
- Check in at the front desk. You will be greeted and asked to sign in.
- You will be asked to complete some forms, or to provide any forms that you received in the mail. If you need assistance, ask the receptionist– we are happy to help you.
- When it is your turn, a nurse or medical assistant will show you into an exam room.
- When your appointment is finished, you will go to the checkout desk. There we will assist you with scheduling future appointments or services, arranging referrals to other providers, and making billing arrangements.
We make every effort to keep the office running on schedule. We ask for your patience when we need some extra time with the appointments ahead of you. If we encounter a long delay, we will try to let you know so that you can adjust your schedule accordingly.
